Guardian Care Management's services are available in Seattle, Tacoma, Bremerton, Olympia and the surrounding areas. We are a preferred resource for locating residential care options. We provide a list of immediate openings at homes with a reputation for only the best care.
Guardian Care Management has knowledge and experience working the Puget Sound long term care facilities. These homes include assisted living residences, adult family homes and nursing homes. Since we do not receive a referral fee from any of the homes, we are able to be an objective resource. We can evaluate your needs or the needs of your loved one to recommend a residence that is most appropriate to the needed level of care. Guardian Care Management can also provide referrals that best fit a person’s need so there is no wasted time calling and filtering through hundreds of different residential options. We enable families to visit only the homes that have a potential to be ideal for their loved one. Our team seeks to provide the best value in a long term care residence.
We also provide care managers who can visit your family member each month to assess care being provided by the facility. Guardian Care Management can a complete report of each monthly visit. This report will contain any changes in condition or concerns about care. If desired, our team can arrange additional activities, visits, or outings for your family member. We hope to enhance your loved one’s stay in a long term care facility by improving their quality of life.

If our client approves, we may act as a liaison to families who live at a distance and provide them with updates on client needs and problems.
